Training – Pre-Engagement Form

Hello,

There are three forms to complete in advance of the training. The first will provide an overview of your current level of knowledge and experience in using participatory photography. Please complete it as comprehensively as you can, and make sure you complete all pages before submitting.

The second is an equal opportunities monitoring form. You do not have to complete this form if you don’t wish to. Please note that this will be submitted anonymously.

The third form is an optional sign up to PhotoVoice’s newsletter. We send these infrequently.

Each form will need to be submitted separately.
